FOM Booster Club Documents

Loads of information about Oak Choir and the 2024/2025 school year can be found in the Handbook, but feel free to ask any FOM Board Member if you have additional questions.

Please read the Handbook thoroughly and electronically sign all forms using DocuSign (email went out to the primary/"adult 1" contact for each family on 1/26/25). Alternatively, you can print, complete and sign a paper copy and return it to the choir room mailbox.

PERMISSION SLIP PACKETS

If you would prefer to complete your permission slip packet with the aid of copy/paste, you can download the appropriate packet to your computer for completion here. You are still responsible for printing, signing, and returning the hard copy of the packet; there is NO E-SIGN OPTION for permission slips.
